Please read the guidance notes before completing this form, and have copies of all supporting documents available. This form needs to be signed by both parents/guardians where possible so that your application can be considered without delay. A home visit to support your application may be requested.

You will need the following documents to accompany your application (if they apply to your situation):

  • Mortgage statement for each property you own
  • Rent agreement/contract/letter
  • Pension letter
  • Loan/credit card statements
  • Payslip, P60, P11D
  • Business accounts, tax return
  • Child benefit (Government letter)
  • Tax credits (Government letter)
  • Income support (Government letter)
  • Housing benefit (Government letter)
  • Carerโ€™s allowance (Government letter)
  • Income from properties (Contract/evidence of related costs)
  • Income from investments (Statements/letters etc)
